We were excited to have Dr Diku Mandavia (Emergency Room Physician Cedars Sinai, Chief Medical Officer SonoSite Inc) , Dr Dina Self (USC ultrasound fellow), & Dr Tom Mailhot (USC ER fellowship director) visit us this week. The doctors are interested in using ultrasound for orthopedic diagnoses in the ER.
The USC Emergency Medicine program is on the cutting edge by providing the latest in technology & clinical research. USC is one of the few programs in the country to offer a Ultrasound Fellowship for Physicians seeking extensive training to provide the highest level of care in a cost effective environment. This visit highlights the steady emergence of Ultrasound in other medical specialties besides it’ s traditional use in Obstetrics.

Dr Steven Sampson was recently interviewed by renowned Orthopaedic Surgeon Brian Cole MD from Chicago, IL on ESPN Radio 1000. Dr Cole is the Section Head of the Cartilage Research Program at Rush University Medical Center and the Cartilage Restoration Center at Rush, specializing in restoration of articular cartilage and meniscus injuries. He actively serves as the team physician for the Chicago Bulls, Co-team doc for the Chicago White Sox, & Depaul University in Chicago.
Dr Steven Sampson was interviewed specifically about how valuable the integration of musculoskjeletal ultrasound has been in clinical practice to rapidly & cost effectively provide accurate diagnosis & treatment. It’s important for people to be aware that this technology is not reserved only for pro athletes that we treat; but i savailable for our patients with everyday pain!

More & more professional sports teams are integrating cutting edge technology to provide the best medical care possible to their patients. Today, Sonosite a manufacturer of handheld ultrasound announced that it’s technology will be exclusively utilized by the NBA.
We have treated NBA players who appreciate this technology because of the inconvenience of fitting a 7 foot plus body frame in a tube & the time delay required to get the study. Now, team doctors & trainers have a point of care option that can quickly access injuries & guide treatment in the locker room. For years elite European soccer clubs have utilized this technology on their players. What many people aren’t aware of is that we use this ultrasound in our practice; not just on professional athletes but also with the general population who suffer with pain. We find that patients enjoy the hands on interactive dynamic associated with ultrasound. Also because we use ultrasound to guide precise injections , patients report less pain. Studies have clearly documented that injections without ultrasound are often not where the physician believes they are, which adversely effects the outcome.

Shoulder Injection Ultrasound demonstration
Dr Steven Sampson was featured in a new iPhone application to teach Doctors across the world how to begin using ultrasound to enhance their practice. Ultrasound is used commonly in Europe, but suprisingly is only now seeing increased use. Ultrasound provides remarkable images of tendons and ligaments and soft tissue in some cases superior to MRI. Also Ultrasound is extremely cost effective compared to other imaging like MRI. Thus by utilizing state of the art technology our health care system could save millions of dollars. “Pioneer patients” are driving awareness to doctors who will have to meet the demand for more cutting edge treatents & imaging to help them recover and heal.

Along with Orthopaedist Ben DuBois MD, Dr Steven Sampson will be hosting a training seminar “Introduction to Shoulder Ultrasound.” The course will be in Seattle WA on June 27, 2009 from 8:30am-5:30pm. Dr Sampson also will be acting as a Preceptor to train physicians who are beginning their ultrasound training.

Yale New Haven Hospital is among the growing community of progressive physicians integrating ultrasound into their orthopaedic practice.

I believe musculoskleletal ultrasound will become a standard tool to help physicians in orthopaedics & sports medicine, much like it is in Europe. With advancements in technology, ultrasound provides unbelievable images of tendons, ligaments, and fluid inside the body. These images are very useful for diagnostic purposesand also guide the physician with injection procedures to ensure accuracy within millimeters of the target. Anecdotally I have found that patients getting cortisone injections have less discomfort with ultrasound guidance because painful structures like bone & tendon are avoided. Also patients enjoy the interaction that ultrasound provides with the physician. Patients can for example move their shoulder around to a particular position to reproduce pain that may not be seen with a static exam like x-ray or MRI. Also ultrasound is cost effective.

A series of studies examined the use, training, outcomes and accuracy of surgeon-held musculoskeletal ultrasound at Yale School of Medicine and Duke Medical School. Researchers found that orthopaedic surgeons can reduce overall time to patient diagnosis, enhance subjective and objective clinical outcomes, and significantly lower some out-of-pocket expense for patients. Results of the studies were presented at the 2008 American Academy of Orthopaedic Surgeons (AAOS) and American Institute of Ultrasound Medicine (AIUM) annual meetings.
