Chronic plantar fasciitis is commonly diagnosed in patients, but can be a difficult condition to treat successfully. Platelet rich plasma (PRP) is the injection of concentrated platelets that is abundant with cytokines (immunomodulation agents) and other growth elements. PRP injection was compared to cortisone injection when treating severe cases of plantar fasciitis that are resistant to the traditional non-operative treatments.
A sample of thirty-six patients was randomized into two study groups and prospectively evaluated. This sample involved 16 males and 20 females who were diagnosed with severe chronic plantar fasciitis and were non responsive to the traditional non-operative treatments such as rest, cam walker immobilization, heel lifts, night splinting, PT, local modalities, and NSAIDS. Before starting the treatment all of the patients had MRI and ultrasound studies which were consistent with plantar fasciitis. The first group was treated with a single 40 mg methylprednisolone ultrasound-guided injection of at the site of the injury. The second group was treated with a single un-buffered autologous PRP ultrasound-guided injection at the site of the injury. For the two weeks of the study the patients were immobilized fully weight bearing in a cam walker. Then they were started on home exercises after which they were allowed to return to their regular activities as their pain would permit without the extra support.
The study was judged based on The American Orthopedic Foot and Ankle Score (AOFAS) which is designed to evaluate the healing process of ankle and foot surgery. The first group originally had the OAFAS score of 52, which increased to 81 after three months, then decreased to 74 after 6 months and further decreased to 58 after 12 months. The second group had original score of 37 which then increased to .95 after 3 months and remained at 94 after 6 and after 12 months following the treatment.
These finding suggests that platelet rich plasma injection is a better and longer-lasting treatment than cortisone injection when treating severe chronic plantar fasciitis.

A recent trend among patients seeking new treatments is a phenomenon that Dr. Sampson, the founder of Orthohealing Center and an expert in the field of orthobiologics and regenerative medicine, has termed “pioneer patients.” Dr. Sampson, at the Sonosite Worldwide Managers Meeting in Palos Verdes, CA, presented his clinical experience to 175 international managers in using bedside musculoskeletal ultrasound. He says that patients are not satisfied with the current status quo as far as the technology available to diagnose and treat orthopedic injuries.

We are seeing a revolution in technology that is being driven by patient to patient marketing. Dr Sampson and the Orthohealing Center were amongst the earliest in the country to integrate this cost effective “hands on” technology and train physicians from all around the world on this and other new treatments utilizing cutting edge technology.
Patients are seeking out further options, believing there must be more treatments medical professionals can offer. We were excited to have Dr Diku Mandavia (Emergency Room Physician Cedars Sinai, Chief Medical Officer SonoSite Inc) , Dr Dina Self (USC ultrasound fellow), & Dr Tom Mailhot (USC ER fellowship director) visit us this week. The doctors are interested in using ultrasound for orthopedic diagnoses in the ER.
The USC Emergency Medicine program is on the cutting edge by providing the latest in technology & clinical research. I was thrilled to participate in an incredibly insightful seminar led by Physician’s from The Mayo Clinic leading the way in Musculoskeletal Ultrasound; Dr Jay Smith & Dr Jon Finnoff. I had the opportunity to meet many leading docs from around the world & exchange protocols and plant seeds for research collaboration. The 3 day course is highly recommended & featured excellent lectures along with workstations with cadavers to practice procedures & diagnostics. It was great to learn how Mayo and other leading centers around the world are integrating state of the art technology with musculoskeletal ultrasound. There were over 1oo Physicians in attendance, a sign that more and more Dr’s are learning this skill to enhance the level of care they provide to their patients.

We are pleased to announce another new peer reviewed publication this yr. In the Feburary 2011 Journal of American Osteopathic Association, Management of Dupuytren Contracture With Ultrasound-Guided Lidocaine Injection and Needle Aponeurotomy Coupled With Osteopathic Manipulative Treatment. Duptyren Contracture is a disabling condition where scar tissue forms on the palm causing restricted range of motion & pain. Surgery is often required but has serious complications and a high recurrence rate with significant recovery time.
Initially I learned about an experimental treatment being done at UCLA to avoid surgery. They were blindly needling the palm and injecting a local anesthetic to break up scar tissue. We took it one step further & published the first known case of ultrasound guided needling for this condition coupled with osteopathic manipulation with manual therapy. With the ultrasound we were able to visualize the irregularity & properly numb the region with little to no pain reported. We were amazed to see a full recovery without any complications. Just another example of how to be on the ‘cutting edge’ without surgery!

Our office is expanding! We’d like to start by thanking all of our valued patients and referring doctors. Nothing gives our clinic more pride than when you refer friends and family members. Because of your support we are expanding our office April 1, 2011.We love our home here in the building on Santa Monica Boulevard, so we are moving to a much larger facility just two flights down in Suite 210. This move will allow us to continue providing state of the art non-surgical orthopedic care while creating a comfortable and warm environment for our patients.
